NOTE: IT IS RECOMMENDED THAT YOU SUBMIT THIS FORM ONE WEEK IN ADVANCE OF YOUR CONCERT DATE.

Submissions from ticket holders (or sincerely wishful thinkers) for each concert only. It is not required to make a selection for every option.

This form is being provided to apprise David of detailed fan preferences in one easy place to simplify his concert decision making, thereby reducing the percentage of disappointed attendees. Results can (though not likely will) then be compiled (volunteers?) for each show, and David will more easily be able to recognize and honor majority preferences in each category, or will be thoroughly confused, the latter being the more probable. David has presumably managed, however unintentionally, to disappoint someone in every single fan demographic. It is hoped that this form might help him to avert such tragedies, or at least minimize them, in the future.

In the event of individual request or option combination ties, David might consider singing a medley of his own songs, make costume and hair changes between songs, e.g., change shirtz/pantz/footwear in descending order of combined request totals, slap on a beanie (or remove it), retain tour hair stylist (or Andy) to add blond highlights or a darker rinse (w/opt. red streaks) to natural auburn base color, add or remove guyliner, etc., several times during a show. This would assure the fewest disappointments.
